Scholarship Renewals for 2011-12 Academic Year

IMPORTANT!

It is the scholarship RECIPIENT’S RESPONSIBILITY to request renewal, and provide required documentation to Community Foundation of the Ozarks by the proper deadline.

Please note all DEADLINE dates may not be the same.

This directory lists renewals for all two, three and four-year scholarships. The amounts are not listed. Please refer to your original award letter for the specific scholarship amount. [Scholarships and amounts subject to change.] If you are renewing multiple scholarships, a Renewal Application must be submitted for each individual scholarship.

Abbott Scholarship – see Stockton Public Schools Foundation Abbott Scholarship

Matt Abraham Memorial Scholarship

Renewals based on amount available to spend (ATS) and maintaining a 3.0 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

The Hearld and Marge Ambler Scholarship

Renewable through the 5th year of schooling. You may request yearly renewal (competitive, not guaranteed). Renewal based on amount available to spend (ATS) by maintaining a 3.0 GPA with continued employment preferred. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Dr. Thomas E. and Mary M. Ashley Medical School Scholarship

You may request yearly renewal, (not guaranteed) if you continue to attend the University of Missouri, and are a student “in good standing” as documented by a signed letter from the Dean or Financial Aid Coordinator. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Dr. Thomas E. and Mary M. Ashley Nursing School Scholarship

You may request yearly renewal, (not guaranteed) if you have maintained a 3.0 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

J. Kenneth Awbery and Iva M. Awbery Scholarship

The 2010-11 Awbery scholarships are non-renewable; prior recipients from 2008-09 may continue to request renewal, as long as you maintain a 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Anita Barnes Memorial Scholarship

The 2010-11 Barnes scholarships are non-renewable; prior recipients for 2008-09 may continue to be renewed.  Based on amount available to spend (ATS), recipients must maintain a 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

The Chris Bell Memorial Scholarship

The 2010-11 Bell scholarship is non-renewable. Based on amount available to spend (ATS); prior recipients from 2008-09 need to refer to your latest award letter for renewal information. You must remain a full-time student i.e. enrolled for at least 12 credit hours and send a one-page update of progress and description of how the Chris Bell scholarship continues to support your career goals, yearly. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Doc and Bonnie Bender Scholarship

The Bender Scholarship amount will vary, renewals are based upon maintaining at least a 2.0 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours and include a one-page update on your academic progress and how the scholarship continues to support your career goals. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

schedule OR no later than June 17, 2011

John C. Bounous Scholarship

The 2010-11 Bounous scholarship is non-renewable

J.K. Burney Scholarship

The 2010-11 Burney scholarship is non-renewable; prior recipients from 2008-09 may continue to request renewal, dependent upon the amount available to spend (ATS), and the recipient’s maintaining a 3.0 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Curtis Rex Carter Science Scholarship

This scholarship award may be renewed, dependent upon the amount available to spend (ATS), and the recipient’s maintaining a 2.8 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

James D. and Mary L. Crafton Scholarship

Prior years’ scholarships may be renewable, one semester at a time; dependent upon the amount available to spend (ATS) and the recipient’s maintaining a 3.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.[IMPORTANT! Greater Ozarks Chapter of National Association of Orthopaedic Nurses Scholarship

You may request yearly renewal, (not guaranteed) depending on availability of funds (ATS), and proof of satisfactory progress. You must remain a full-time student. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Thomas Halbert Scholarship

You may request yearly renewal, (not guaranteed) depending on availability of funds (ATS), and the recipient’s continued enrollment in a Springfield, MO nursing school with proof of satisfactory progress. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

Geraldine Haldiman Scholarship

The 2010-11 Haldiman scholarship is non-renewable.

Hanks-Wilson Ozark High School Scholarship

This scholarship may be renewed for one additional year, at approximately half the amount of the initial award. See your original award letter for scholarship amount. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Ozarks Teacher Corps

A two-year scholarship, renewal is dependent upon the amount available to spend (ATS), and  continuing to meet your respective department of education’s certification and graduation requirements. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Ruby Knutson and Esther Miles Scholarship

The 2009-10 Knutson/Miles scholarships are non-renewable; prior recipients from 2008-09and 2010-11 may continue to request renewal, as long as you maintain a 2.0 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

John H. and Ruby M. Laurie Charitable Fund

The 2010-11 Laurie scholarships are non-renewable. A two-year scholarship, Laurie amounts are variable. The recipient must maintain a 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

* Mark’s Memorial Scholarship

The 2010-11Mark’s Memorial scholarship is non-renewable.

Dr. Joe McAdoo Southwest Missouri Public Relations Society (PRSA) Scholarship

You may request yearly renewal, (not guaranteed) if you have maintained satisfactory progress. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Dennis Miller Memorial Scholarship

This scholarship is renewable for three (3) years providing recipient maintains a 3.0 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Andrew R. Norton Memorial Scholarship

The 2010-11 Norton scholarship is non-renewable. This is a 2-year scholarship and is renewable for that second year, providing you maintain a 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

The Patriot Family Scholarship

This scholarship is renewable for an additional three (3) years, providing recipient maintains 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Barker Shannon Memorial Scholarship

You may request yearly renewal, (not guaranteed) if you have maintained satisfactory progress. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Ann and Doug Sickles Memorial Scholarship

You may request yearly renewal, (not guaranteed) for up to three (3) years, depending upon amount available to spend and maintaining a 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Smith-Glynn-Callaway Foundation Scholarship in Honor of Dr. Stanley Peterson

A four-year scholarship, renewals are dependent upon recipient’s satisfactory academic achievement. You must submit a letter “in good standing” from your Dean of Students or Financial Aid Coordinator. Once your completed Renewal Form and accompanying documentation is received and approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011.

Ste. Genevieve County Community Foundation Scholarship

This scholarship is renewable for three (3) years, providing you maintain a cumulative GPA of a 3.0. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

St. John’s Hospital School of Nursing Alumni Scholarship

You may request yearly renewal, (not guaranteed) if you have maintained a 3.0 GPA and continued full-time enrollment. Once your completed Renewal Form and accompanying documentation is received, if approved, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

Dorman L. Steelman Memorial Scholarship

A two-year scholarship, renewals are based on amount available to spend (ATS) and maintaining a 2.5 GPA. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

Stockton Public Schools Foundation Abbott Scholarship

The 2010-11 Abbott scholarship is non-renewable.

Stockton Public Schools Foundation Korth Scholarship

The 2010-11 Korth scholarship is non-renewable. Prior recipients from 2008-09 may continue to request renewal, as long as you maintain satisfactory academic progress. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Henry J. and Lucille J. Straus Scholarship

The Straus Scholarship is renewable for three (3) additional years, You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Henry Joseph Struck Memorial Scholarship

Prior recipients may continue to request renewal for three (3) years as long as you maintain a 3.0 GPA, and submit a yearly written narrative describing what you have learned about yourself through your college experience and/or how you have changed/grown during that academic year. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

* Arthur G. Tesar Scholarship

The 2011-11 Tesar scholarship is non-renewable. Students from 2008-09 may continue to request renewal. Renewals must maintain a 3.0 GPA and full-time enrollment of  at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

DEADLINE: as soon as you have your current semester’s grades, and next semester’s schedule OR no later than June 17, 2011

Bill Wallis Scholarship

The Wallis Scholarship varies in renewal amounts (see your original award letter.) Regardless of which scholarship you were awarded, you must maintain a cumulative GPA of 2.8. As an employee, dependent of employee, or dealer/vendor, you must be actively employed or doing business with Wallis Companies at the time your grades are submitted; unless you were an employee who is now attending college on a full-time basis. If your parent is no longer an employee/dealer when 2.8 GPA is submitted, the decision to renew will be determined on a case by case basis by the selection committee. If you do not intend to renew the Wallis Scholarship consecutively during a four year period, special permission must be requested from the selection committee prior to the beginning of the academic year. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.

Glen W. Wisdom Scholarship

A two-year scholarship, Wisdom is renewable the second year providing the student maintains satisfactory academic progress. You must remain a full-time student i.e. enrolled for at least 12 credit hours. Once your completed Renewal Form and accompanying documentation is received, payment will be made to your student account and scholarship monies applied ½ Fall semester, ½ Spring semester, for the upcoming academic year.
