This fund provides support for Barton and Dade County families with children with disabilities who are under the age of 18. Applications for assistance will be individually evaluated by a committee after completion of the form and verification from your health care provider concerning your child’s diagnosis. Information provided in this application is strictly confidential and will be used only for the purpose of grantmaking by the Community Foundation of the Ozarks.
